RECERTIFICATION STEP 2:

MATCH EXPERIENCE

You must prepare your Match Experience Form for your records.
I have posted 2 versions online. Any method or combination of the methods below will be acceptable:

• Microsoft Word-Allows you to fill it in on your computer and print it out.
• Adobe Acrobat-You must fill in by hand.
• Arbiter Printout signed and certified ("I certify this document is correct," and sign your name.)

THIS FORM MUST BE SIGNED AND DATED.

This form was also included in last year's recertification booklet.

If you do not fill this form in and SIGN IT, you will not get credit for the games you did!

REMEMBER-Only USSF AFFILIATED GAMES COUNT for upgrade from 8 or 9 to 7. Only FULL LENGTH USSF AFFILIATED GAMES COUNT for upgrade beyond that point.

In our area that would be:

State Cup, State Cup Playdowns, Upstate Premier, Elmira Adult League, Broome County Soccer Association Spring League, Tompkins County Fall League and any USSF outdoor tournaments. Indoor matches do not count unless they have been specifically approved by the state association and/or USSF.
